Deep Roots Consent Forms

  • Consent forms must be filled out on the day of your appointment, before you come in. They are required for every procedure. If you fill it out prior to the day of your appointment, you will need to complete it a second time.
  • If you are an adult or a minor receiving a piercing, please use the General Piercing Consent Form below.
  • If you are an adult or a minor receiving an alteration to your piercing (installation/alteration/removal) or need a check up, please use the Piercing Alteration Form below.
  • You must fill out the correct form in order for us to proceed with your appointment. If you have any questions, please contact us in Seattle or Lynnwood!
